Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to provide in-home therapy for patients with speech, language, swallowing, and cognitive disorders. The role entails delivering coordinated care in patients’ residences, with a focus on restoring or maintaining well‑being in a home health setting.

Requirements include a Master’s in Speech Language Pathology, current state licensure, ASHA CCC, a valid driver’s license, and at least 2 years’ experience in acute care or rehabilitation

Qualifications

Master’s degree in Speech Language Pathology from an accredited college or university.
Current state license in Speech Language Pathology.
Certificate of Clinical Competence from American Speech Language Hearing Association.
Valid driver’s license.
2 years’ experience in an acute care or rehabilitation setting, home health preferred.
Reliable transportation with current auto liability insurance.

Responsibilities

Provides coordinated care to patients in their place of residence.
Plans, implements, and evaluates patient care plans to restore or maintain patient well‑being.
Collaborates with interdisciplinary team.
