Job description

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for a Per Diem opportunity providing Home Health visits across Cuyahoga County. This role serves adult and geriatric patients in their homes.

Prior home care experience as an SLP is a plus but not required.

As an SLP in home care, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling patients to receive necessary medical care at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Conduct evaluations to assess, diagnose, and treat speech, language, communication, or swallowing issues.
3. Develop treatment plans based on evaluations, physician’s orders, and goals.
4. Regularly assess and update care plans; collaborate with the care team as needed.
5. Educate patients and families on rehabilitative activities to promote health and independence.
6. Document observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

1. Current Ohio licensure as a Speech Language Pathologist.
2. Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or equivalent, or be eligible and awaiting certification exam.
3. Completed relevant education and work experience as required for certification.
4. Strong computer skills; experience with electronic medical records preferred.
