Job description

BAYADA Home Health Care

is currently seeking an experienced Speech Language Pathologist (SLP) for a Per Diem opportunity performing Home Health visits throughout areas of Cuyahoga County.

Our office services adult and geriatric patients in their homes. Prior home care experience as an SLP is a plus but not required. As a home care SLP, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
3. Develop and revise treatment plans based on evaluations, physician's orders, and goals.
4. Collaborate with BAYADA clinicians in multidisciplinary care planning.
5. Educate patients and family members on rehabilitative care to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic devices.

Qualifications:

1. Current Ohio license as a Speech Language Pathologist.
2. Certificate of clinical competence from the American Speech-Language-Hearing Association (ASHA) or equivalent educational and work experience, with pending certification examination.
3. Strong computer skills; experience with electronic medical records (EMR) preferred.
