Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team. The SLP will assess referred patients needing rehabilitation and develop a treatment plan aligned with physician direction.

The role emphasizes documentation, progress notes, and discharge summaries. Experience in geriatric care is preferred; there is a sign-on bonus and highly competitive rates in a long-term care setting.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and set realistic, achievable goals.
• Establish and administer treatment programs under physician direction.
• Document patient condition and educate patients and families.
• Assist with evaluations, progress notes, recertification and discharge documentation.
