Job summary

Nexus at Godfrey is seeking a Speech Language Pathologist to evaluate and treat residents with communication, cognitive, and swallowing disorders, and to develop individualized treatment plans to improve safety and quality of life.

The role includes implementing AAC strategies, educating clients and caregivers, and collaborating with medical staff on swallowing procedures. Illinois licensure and experience in long-term care are preferred.

Qualifications

  • Active/Valid Illinois license in Speech-Language Pathology.
  • SNF/Long term care experience preferred.
  • Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

  • Provide skilled, medically necessary speech and language therapy assessments and develop individualized treatment plans.
  • Provide AAC systems for individuals with expressive and/or language comprehension disorders.
  • Train and educate clients, families, caregivers, and other professionals as subject matter experts.
  • Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physicians.

Position Overview:

Evaluates and treats residents with communication, cognitive, and swallowing disorders to help improve safety, independence, and quality of life. The SLP develops individualized treatment plans to address speech, language, cognition, voice, and swallowing needs.

Key Responsibilities:
  • Provide skilled, medically necessary speech and language therapy assessments as well as developing and implementing individualized treatment plans to increase client’s independence and functioning.
  • Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
  • Provide training and education to clients, family, caregivers, and other professionals as subject matter experts.
  • Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physicians.
