Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our dynamic SNF team. You will evaluate and treat patients by following an individualized plan of care based on comprehensive assessments.

Applicants with experience in standardized speech/language assessments, swallowing evaluations, and developing treatment plans will work closely with patients, families, and the treatment team to monitor progress and optimize outcomes.

Qualifications

• Experience administering standardized speech-language assessments.
• Ability to conduct swallowing evaluations.
• Developing individualized treatment plans and monitoring progress.
• Strong written and verbal communication with patients, families, and other treatment team members.

Responsibilities

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
