Job summary

WarsawRehabilitation And Healthcare Center in Warsaw, NC is seeking a Speech Language Pathologist to join our long-term care team. The role includes comprehensive speech therapy evaluations, developing treatment plans, and coordinating with physicians and care staff.

Qualifications

• Graduate of an accredited university with an MA, or MS in communication disorders recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations per MD orders.
• Document findings on standardized evaluation formats accurately.
• Develop comprehensive treatment plans with long/short term goals, frequency, and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Provide treatment using indicated modalities and adjust as needed while adhering to guidelines.
• Document progress and discharge summaries in approved formats.
