Job summary

Asheboro Rehabilitation And Healthcare Center in Asheboro, NC, a Long Term Care facility, is hiring a Speech Language Pathologist PRN to join its growing rehab team. The role involves evaluating and treating residents, adhering to MD orders, and maintaining thorough documentation.

Qualifications

• Graduate of an accredited university with an MA, or M.S. in communication disorders recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations per MD orders.
• Document findings on the evaluation format neatly and accurately.
• Develop a comprehensive treatment plan with goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment using appropriate modalities and adhere to safety guidelines.
• Document patient progress in approved formats and in a timely manner.
• Prepare monthly updated treatment plans in standardized format.
• Document discharge and in-service as indicated.
• Complete all required documentation by the facility.
• Assist with care plans and information for the MDS as needed.
• Participate in facility and rehab meetings as indicated.
• Act as rehab delegate to the interdisciplinary team.
