Job summary

Napa Valley Care Center in Napa, CA is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team. You will evaluate residents with speech, language, cognitive, and swallowing disorders and develop individualized treatment plans in collaboration with physicians and staff.

Qualifications

• Master's or Doctoral Degree in Speech-Language Pathology.
• Active and unrestricted California Speech-Language Pathologist License.
• Current license maintained in good standing.
• Strong clinical, documentation, and communication skills.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Evaluate residents referred for speech, language, cognitive, and swallowing disorders within established timeframes.
• Develop individualized treatment plans and obtain physician approval as required.
• Provide skilled speech-language pathology services in accordance with physician orders and clinical standards.
• Assess and treat communication, cognition, voice, language, speech, and dysphagia disorders.
• Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to achieve optimal outcomes.
• Participate in care plan meetings, patient care conferences, and interdisciplinary team discussions.
• Educate residents, caregivers, families, and staff regarding treatment goals and therapeutic techniques.
• Complete timely, accurate, and compliant documentation in electronic medical record systems.
• Assist with discharge planning and caregiver training.
• Supervise Speech-Language Pathology Assistants and CFs when applicable.
• Support ongoing staff education through in-service training and mentorship.
