Job summary

Country Oaks Care Center in Pomona, CA is seeking a Speech Language Pathologist/CFY for a full-time position. You will conduct clinical evaluations, develop treatment plans, and educate families to support optimal patient outcomes.

CFY candidates are welcome; state licensure is required and CPR certification is preferred. The role emphasizes collaborative, cross-disciplinary care and quality patient care in a post-acute setting.

Qualifications

• Master of Science in Speech/Language Pathology required.
• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Speech/Language Pathology experience preferred.
• 1-2 years’ experience in a residential/assisted living or skilled nursing facility preferred.
• Students in their CFY (Clinical Fellowship Year) will be considered.

Responsibilities

• Provides, directs and supervises all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Ensures patient care follows guidelines and professional standards and complies with Federal and State regulations.
• Acts as patient/rehabilitation advocate and coordinates with cross-disciplinary teams.
• Participates in quality assurance and customer service activities.

Provides, directs and supervises all aspects of quality patient care, including, but not limited to; screening, evaluation and treatment, treatment planning, goal setting, family education and documentation, in order to assure the best outcome possible for the patient. The Speech Language Pathologist (SLP) assures that the patient care follows the organization’s guidelines; professional standards and community needs, and is in compliance with Federal and State guidelines.

The SLP serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers. Ensures cross-disciplinary coordination in order to provide for consistent patient care. Participates in quality assurance and customer service activities.
