Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in Anderson and Fishers, IN, visiting homes or Senior Living Communities.

Qualifications

• Valid SLP license in IN or eligibility to obtain.
• Degree from accredited SLP program.
• Current CPR certification.
• Current ASHA CCC.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults wherever they call home (private residences, senior living communities, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access and flexibility.

Qualifications

Physical Requirements

• FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE whenrequired
