Job summary

Southview Acres Healthcare Center, a Skilled Nursing Facility in Minnesota, seeks a PRN In-House Speech-Language Pathologist to assess, treat, and educate residents in collaboration with the interdisciplinary team. Qualifications

• Must have completed an accredited Speech-Language Pathology program.
• Valid state license/registration in Minnesota required.
• Experience in long-term care or geriatric settings preferred.
• Proficient in EMR documentation and medical record-keeping.
• Collaborates with multidisciplinary team to deliver patient-centered care.
• Excellent patient service and clinical excellence.

Responsibilities

• Set realistic goals, document progress, and educate patients and families about care plans.

Southview Acres Healthcare Centeris looking for a PRN In-House Speech/Language Pathologist to become a part of our team!

Responsibilities Include:

• The Speech Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.

• The Speech Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.

• The Speech Therapist will set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families with regard to the plan of care.

Qualifications/Requirements:

• Graduated from an accredited Speech Therapy college program or university

• Valid professional license and registration for the state of MN

• Experience with long-term care and/or geriatric care is preferred but not limited to

• Well-versed with EMR documentation Evaluations, weekly progress notes, recertification, and discharge documentation

• Collaborates with all disciplines to provide the best patient-centered treatment and care

• Exceptional patient service skills and ability to demonstrate clinical excellence

• Adhere to scheduled hours and demonstrate professionalism at all times

• Perform all other duties assigned by the Director of Rehabilitation Compliance with confidentiality & HIPAA
