Job summary

Adept Nursing & Rehab of Waverly is seeking a compassionate Speech Language Pathologist (SLP) to join our interdisciplinary team in a skilled nursing facility. This PRN role focuses on helping residents regain communication, swallowing, and cognitive abilities to improve independence.

You will evaluate, diagnose, and treat residents, develop individualized plans, and collaborate with a multidisciplinary team to support patient-centered outcomes.

Qualifications

• Master’s Degree in Speech-Language Pathology.
• Current state licensure as a Speech-Language Pathologist.
• CCC-SLP preferred or Clinical Fellow welcome to apply.
• Experience in skilled nursing facility, rehabilitation, or geriatric setting preferred.
• Strong communication, organizational, and documentation skills.

Responsibilities

• Evaluate, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders in residents
• Develop and implement individualized treatment plans based on patient needs and physician orders
• Provide therapy to residents recovering from stroke, neurological conditions, surgery, or illness
• Assess and manage dysphagia and recommend appropriate dietary modifications
• Document patient progress and maintain accurate clinical records in accordance with regulatory and facility guidelines
• Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and other interdisciplinary team members
• Educate residents, families, and staff on therapy techniques and treatment plans
• Participate in care planning meetings and contribute to patient-centered outcomes
