The Grand Rehabilitation and Nursing at Barnwell in Valatie, NY is seeking a Speech Language Pathologist to join our interdisciplinary team. Job Summary:

The Speech Language Pathologist (SLP) will evaluate and treat residents with speech, language, cognitive, voice, and swallowing disorders. This role will collaborate closely with nursing, therapy, and medical teams to support safe communication and nutrition outcomes in a skilled nursing environment.

Primary Responsibilities:

• Evaluate residents for speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide therapy to improve communication, cognition, and safe swallowing
• Document evaluations, progress notes, and discharge summaries accurately
• Ensure compliance with state, federal, and facility documentation standards

Qualifications:

• Current NYS license as a Speech Language Pathologist
