Job summary

H2 Health is seeking a Part-Time Speech-Language Pathologist to provide 1:1 patient care for geriatric clients in senior living communities.

The role emphasizes dysphagia management, cognitive support, and interdisciplinary collaboration. Candidates should hold CCC-SLP licensure and have outpatient therapy experience. Flexible scheduling with no weekends or holidays is offered.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Active CCC-SLP state license is required.
• Experience with dysphagia is required.
• Outpatient therapy experience is a plus.

Responsibilities

• Evaluate and treat a predominantly geriatric 1:1 caseload.
• Address dysphagia, cognitive impairment and neurologic disorders in therapy.
• Work independently and as part of an interdisciplinary team.
• Make clinical decisions based on patient functional needs.

This position will evaluate and treat a primarily geriatric caseload 1:1 with a mix of dysphagia, cognitive impairment, neurologic disorders, and problem-solving programs. The therapist must be able to work independently and in teams and make appropriate decisions based on each patient's functional needs.
