Job summary

Surprise Health & Rehabilitation in Surprise, AZ is seeking a dedicated Full Time Outpatient Speech-Language Pathologist. You will assess patients, develop individualized treatment plans, and provide therapy across settings including skilled nursing facilities, independent living, and assisted living facilities.

OUTPATIENT SPEECH-LANGUAGE PATHOLOGIST - FULL TIME -

SURPRISE HEALTH & REHABILITATION - SURPRISE, AZ

We are seeking a dedicated and compassionate Full Time Outpatient Speech-Language Pathologist to join our team at Surprise Health & Rehabilitation. This position offers an excellent opportunity to make a meaningful impact on patients’ lives by providing therapy services in a variety of settings, including skilled nursing facilities, independent living, and assisted living facilities. You will treat patients in both inpatient and outpatient settings, often within the comfort of their homes.

Full Time: Flexible schedule with work-life balance

Wage: $40 - $50/hour based on experience

Key Responsibilities:

• Conduct assessments and develop individualized treatment plans.
• Provide therapy services to patients in skilled nursing facilities, independent living, and assisted living facilities.
• Monitor patients’ progress and document their responses to treatment.
• Collaborate with a multidisciplinary healthcare team to provide comprehensive patient care.
• Educate patients and their families on treatment plans and home exercise programs.
