Job summary

Flagship Therapy is hiring a Speech-Language Pathologist for a full-time role at Lakeshore Post Acute and Rehabilitation Center in Pueblo, CO. The position offers a stable schedule, supportive in-house rehab team, and compensation of $40–$55/hour based on experience.

Key duties include screening and evaluating patients, delivering rehabilitative and medically necessary treatments, and documenting discharge summaries. A current SLP license is required; new graduates are encouraged to apply.

Responsibilities

• Screen and evaluate patients, develop care plans.
• Deliver rehabilitative, medically necessary treatment per standards.
• Stand and walk throughout the facility; assist with transfers.
• Administer procedures as part of the rehab plan (manual techniques, exercises, wound care).
• Provide counseling to patients, families, and other providers.
• Generate discharge summaries meeting regulatory requirements.

Lakeshore Post Acute and Rehabilitation Center in Pueblo, CO, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time Speech-Language Pathologist for our in-house rehab program!

Full Time: Flexible schedule with work-life balance

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Speech-Language Pathologist license is required.
• Open to SLPs at all experience levels. New Grads are welcome!
