Job summary

Fairview Rehab and Care in Philadelphia, PA is seeking a Speech Language Pathologist to join our team on a full-time basis. The role involves direct patient care, assessment, and development of treatment plans in collaboration with physicians and other health professionals.

Strong communication, organization, and compassion are essential. PA licensure is required, and experience in long-term or geriatric care is preferred. PRN availability may also be considered.

Qualifications

  • Must have graduated from an accredited Speech Therapist college or university.
  • Must have a valid PA license and registration.
  • Experience with long-term care and/or geriatric care is preferred.

Responsibilities

  • Assess patients requiring rehabilitation services.
  • Provide direct patient care and determine goals based on patient capacity.
  • Establish and administer treatment plans with physician direction.
  • Set realistic, achievable goals and document patient progress.
  • Educate patients and families on therapy methods.
  • Complete evaluations, progress notes, recertifications and discharge documents.
  • Collaborate with all disciplines for patient-centered care.
  • Perform duties assigned by the Director of Rehabilitation.
  • Maintain confidentiality and residents' rights.

Duties and Responsibilities:

Assessment of patients that require rehabilitation services

  • Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
  • Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
  • Set realistic and achievable goals for their patients
  • Document and record the patients’ condition
  • Educate patients and families in an appropriate therapy method.
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
Qualifications/Requirements:

Must have graduated from an accredited Speech Therapist college or university.

  • Must have a valid professional license and registration for state of PA
  • Experience with long-term care and/or geriatric care is preferred but not limited to.
