Job summary

Plaza Community Living Center is seeking a compassionate Speech-Language Pathologist to deliver high-quality speech, language, communication, and swallowing services. The role includes assessments, treatment planning, and collaboration across two facilities to support residents.

Licensed SLP with strong clinical skills and excellent communication will work with a multidisciplinary team, ensuring thorough documentation and ongoing patient progress in a supportive environment.

Qualifications

  • Current Speech-Language Pathologist license/certification as required by the state.
  • Strong clinical knowledge and assessment skills.
  • Ability to work effectively with patients with a variety of needs and personalities.
  • Excellent interpersonal and communication skills.

Responsibilities

  • Assess, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders.
  • Develop and implement individualized treatment plans based on each patient’s needs and goals.
  • Conduct screenings and evaluations to identify patients who may benefit from speech therapy services.
  • Monitor patient progress and modify treatment plans as needed.
  • Collaborate with nursing staff, physicians, and other healthcare professionals to provide comprehensive patient care.

  • Maintain accurate and timely documentation in accordance with facility and regulatory requirements.
