Position Overview
The PRN SLP will provide high‑quality speech, language, cognitive, and swallowing therapy services to residents in a skilled nursing setting. This role requires adaptability, strong clinical judgment, and collaboration with an interdisciplinary team to deliver exceptional patient care.
Responsibilities
- Evaluate, treat, and document speech, language, cognitive, and swallowing disorders in accordance with physician orders and plan of care.
- Develop and implement individualized treatment plans.
- Maintain accurate, timely documentation in compliance with regulatory standards.
- Collaborate with nursing, therapy team members, physicians, and families.
- Participate in care planning and interdisciplinary team meetings.
- Screen residents for communication problems and conduct in‑depth evaluations to make written recommendations for a therapy plan of care.
- Coordinate audiometric screening tests and refer for further audiology evaluations or treatments if necessary.
- Initiate speech‑language therapy plan of care according to physician orders and maintain required contact with the physician during the therapy program.
- Observe residents’ rights and treat residents with respect and kindness.
- Maintain written evaluations, progress notes, and discharge summaries for all residents in a speech‑language treatment program.
- Confere with other staff regarding special needs of residents and integrate the speech‑language treatment program with goals and approaches of other disciplines.
- Attend resident care planning and family/resident care plan review meetings for residents currently in a speech‑language treatment program.
- Maintain records including statistical reports about residents, in‑service education information, budget information, and material/equipment inventory.
- Submit statistical reports to the Business Office staff for Medicare billing payments and complete assessment forms upon request.
- Attend staff meetings and required or preferred in‑service training.
- Participate in developing and conducting in‑service training programs for facility personnel involved in resident care.
- Review written speech‑language policies and procedures annually and make necessary revisions.
- Maintain costs of the program within the parameters of the annual budget.
Qualifications
- Current New York State license as a Speech‑Language Pathologist.
- M.A. or M.S. Degree in Speech‑Language Pathology from an approved school required.
- Certificate of Clinical Competence (CCC‑SLP) preferred; Clinical Competence Certification from ASHA preferred.
- Experience in a skilled nursing facility preferred.
- Strong clinical, organizational, and communication skills.
- Flexibility to work as needed based on facility demands.
- Ability to work independently and as part of a team.
- Current, valid CPR/BLS credential.
- Ability to communicate well verbally and in writing required.
- Ability to read, write, speak, and understand English at an intermediate or more advanced level.
- Verification of qualifications by the State Department of Health.
- Rehabilitation Therapies license number/certification number required.
