Qualifications

• Experience in Skilled Nursing Facility or long-term care preferred.

• Conduct evaluations and re-evaluations for residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans aligned with resident goals and clinical findings.
• Collaborate with nursing, dietary, and therapy teams for coordinated care delivery.
• Document all services per federal, state, and facility guidelines.
• Educate residents, families, and staff on communication strategies and dysphagia precautions.

Location: Bronx, NY

Pay Rate: $55-$63 per hour

Schedule: Full-time
