Job summary

Deercreekseniorliving is looking for a Speech Language Pathologist (SLP) to provide therapy services in the homes of patients, ensuring compliance with physician orders and the care plan under the supervision of the Director of Rehab.

The successful candidate will enhance patient communication, manage evaluations and history taking, and work closely with healthcare teams to maximize patient outcomes.

Qualifications include a clinical competency certificate in speech pathology and two years of experience preferred, as well as a valid driver's license and access to transportation.

Qualifications

• Ability to drive for agency-related travel is essential.

Job Summary

Speech Language Pathologist (SLP) administers speech therapy to patients in their place of residence. This is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Director of Rehab.

Duties & Responsibilities

• Improves or maximizes the communication of the patient.
• Periodically participates with all other health care personnel in patient IDG care planning.
• Provides full range Speech Language Pathology Services as ordered by physician and IDG.
• Takes initial history and makes initial evaluation.
• Performs all procedures.
• Consults with physicians and IDG regarding change of treatment.
• Instructs patients and family members in home programs.
• Periodically presents an in‑service to hospice staff members.
• Assists the physician and IDG in evaluating level of function.
• Prepares clinical and progress notes.
• Advises and consults with the family and other hospice personnel.
• Participates in in‑service programs.

Job Requirements

• Certificate of Clinical Competency in Speech Pathology granted by the American Speech‑Language‑Hearing Association, or being in the process of accumulating supervised experience for certification.
• Two (2) years’ experience preferred.
• Currently licensed in the state(s) in which practicing.
• Ability to drive and/or have access to transportation for agency‑related travel (e.g., community partners, patients, etc.) is an essential function of the job.
• Must have and maintain a valid driver’s license, maintain automobile insurance coverage, and have access to an automobile.
