Job summary

Symbii Home Health and Hospice-Layton is seeking a Speech Language Pathologist (SLP) to administer speech therapy to patients in their homes in Kentucky, under physician orders and the care plan, and supervised by Rehab Director.

Responsibilities include assessments, therapy, care planning with IDG, documenting notes, family instruction, and collaboration with physicians; must meet CCC-SLP or be in process; licensure and transportation required.

Qualifications

  • CCC-SLP or in process to certification with supervised experience.
  • Current licensure in applicable state(s).
  • Two years' experience preferred in home health or related setting.

Responsibilities

  • Administers speech therapy to patients in their homes under physician orders and care plan.
  • Implements full range speech language pathology services as ordered.
  • Takes initial histories and evaluations, prepares notes and progress reports.
  • Consults with physicians/IDG about treatment changes.
  • Instructs patients and families in home programs.
  • Participates in IDG care planning and in-service presentations.
  • Evaluates level of function and documents care progress.
  • Maintains required documentation and records.

JOB SUMMARY


A Speech Language Pathologist (SLP) administers speech therapy to patients in their place of residence. This is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Directorof Rehab.


DUTIES & RESPONSIBILITIES



  • Improves or maximizes the communication of the patient.

  • Periodically participates with all other health care personnel in patient IDG care planning.

  • Provides full range Speech Language Pathology Services as ordered by physician and IDG.

  • Takes initial history and makes initial evaluation.

  • Performs all procedures.

  • Consults with physicians and IDG regarding change of treatment.

  • Instructs patients and family members in home programs.

  • Periodically presents an in-service to Hospice's staff members.

  • Assists the physician and IDG in evaluating level of function.

  • Prepares clinical and progress notes.

  • Advises and consults with the family and other Hospice personnel.

  • Participates in in-service programs.

JOB REQUIREMENTS(Education, Experience, Knowledge, Skills & Abilities)



  • A person who meets the education and experience requirements for a certificate of Clinical Competency in Speech Pathology granted by the American Speech Language Hearing Association, or

  • A person who meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certification.

  • Two (2) years' experience, preferred.

  • Currently licensed in the state(s) in which practicing.

  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.
