Job summary

The Pennant Group, Inc. in Oregon is seeking a Speech-Language Pathologist to implement standards of care for speech-language pathology services in home health settings.

Responsibilities include evaluating patients, developing individualized care plans with caregivers, providing physician-ordered therapy, educating patients and families, preparing notes, and coordinating with physicians and other practitioners.

Qualifications

  • Must have a master’s or doctoral degree in speech-language pathology and be licensed in the state where practicing.
  • If licensure is not required, complete 350 clock hours of supervised clinical practicum and at least nine months of supervised work.
  • Demonstrates good verbal and written communication, and organization skills.

Responsibilities

  • Conduct evaluations, assess home environment, and identify equipment needs for speech/language function.
  • Develop an individualized plan of care with patient, representative, and caregiver.
  • Provide physician-ordered services and patient/caregiver education.
  • Prepare clinical notes and communicate with the physician and other health care practitioners.
  • Identify functional speech defects and establish a plan to improve patient function.
  • Administer speech therapy using specialized techniques and/or equipment.
  • Recommend communication devices/aids as indicated.
  • Participate in case conferences, in-services, and other meetings for coordinated care.
  • Maintain clinical competency in speech language pathology practice and theory.
  • Refer patient needs to other services as appropriate; supervise home health aide when applicable.
  • Contribute to quality assessment and performance improvement programs.
  • Perform other duties to facilitate speech language pathology services.

SPEECH-LANGUAGE PATHOLOGIST JOB DESCRIPTION
1.JOB SUMMARY

The Speech–Language Pathologist is responsible for implementation of standards of care for speech language pathology services.

2.DUTIES & RESPONSIBILITIES
  1. Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment
    needs relative to speech/language function.
  2. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized
    plan of care in partnership with the patient, representative (if any), and caregiver(s);
  3. Provides services that are ordered by the physician as indicated in the plan of care;
  4. Provides patient, caregiver, and family counseling;
  5. Provides patient and caregiver education;
  6. Prepares clinical notes;
  7. Communicates with the physician who is responsible for the home health plan of care and other health care
    practitioners (as appropriate) related to the current home health plan of care
  8. Identifies functional speech defects and establishes a plan of care to improve patient’s function.
  9. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  10. Recommends communication devices/aids as indicated.
  11. As appropriate, participates in case conferences, in-services and other meetings required ensuring coordinated
    and comprehensive care.
  12. Maintains clinical competency in speech language pathology practice and theory.
  13. Identifies patient and family/caregiver needs for other services and refers as appropriate.
  14. May supervise home health aide in procedures directly related to the speech–language therapy plan of care. 15. Participates in the HHA’s quality assessment and performance improvement program and HHA-sponsored in-
    service training.
  15. Performs other duties as required to facilitate the delivery of speech language pathology services.

3.JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
  1. Must have a master’s or doctoral degree in speech-language pathology and be licensed in the state where
    practicing. If licensure is not required, the speech-language pathologist must have completed or be in the process of completing 350 clock hours of supervised clinical practicum (or is in the process of accumulating supervised clinical experience). In such case, the speech-language pathologist is to have performed not less than nine months of supervised full-time speech-language pathologist services after obtaining a master’s or doctoral degree in speech-language pathology or a related field; and successfully completed a national examination in speech-language pathology approved by the Secretary.
  2. A minimum of one year’s clinical experience in Speech–Language Pathology.
  3. Demonstrates good verbal and written communication, and organization skills.
  4. Possesses and maintains current CPR certification.
  5. The ability to drive and/or have access to transportation for agency related travel (e.g. community partners,
    patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
4.PHYSICAL REQUIREMENTS
  1. Employees are asked to sign a statement indicating whether they can perform the essential functions of their

  2. Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions
    when kneeling, stooping, bending or reaching may be necessary.
  3. The employee must be able to lift and/or move up to 15 lbs on a regular basis. There may be occasions in
    which the employee must be able to lift or move up to 25 lbs.
  4. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision,
    depth perception and ability to adjust focus.
