Position Overview:

Fountains at Mill Cove is seeking a compassionate and skilled Per Diem Speech-Language Pathologist to join our interdisciplinary team at our in-house subacute rehabilitation facility in Jacksonville, FL. The ideal candidate will provide high-quality, patient-centered care to individuals recovering from illness, injury, or surgery, helping them regain their communication, cognitive-linguistic, and swallowing abilities.

Key Responsibilities:

• Conduct comprehensive evaluations of patients with speech, language, cognitive-communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on evaluation findings and patient goals.
• Provide therapy services in accordance with best practices and facility policies.
• Document patient progress and outcomes accurately and in a timely manner using EMR systems.
• Collaborate with a multidisciplinary team, including physicians, nurses, physical and occupational therapists, and case managers, to support optimal patient recovery.
• Educate patients, families, and caregivers on treatment strategies and carryover techniques.
• Participate in discharge planning and recommendations for continued care as appropriate.
  
  
  

Qualifications:

• Current Florida SLP license (or eligibility)
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; CFY candidates may be considered
• Prior experience in subacute rehab, skilled nursing, or post-acute care preferred
• CPR certification (or ability to obtain)
• Strong clinical skills and ability to work independently
• Excellent communication, organizational, and interpersonal skills
