Job summary

Discovery at Home, part of the Discovery Senior Living family, seeks a Physical Therapist for the Sun City, FL area in a PRN role. You will evaluate patients, develop and adjust treatment plans, monitor progress, and educate patients and caregivers to improve mobility and reduce injuries.

The role requires licensure in Florida, a degree in Physical Therapy, and a valid CPR certification. On-site visits with flexible scheduling.

Qualifications

• Requires a degree in Physical Therapy from an accredited program.
• Must be licensed or registered by the applicable state without restrictions.
• One year of home health experience required.
• Must have knowledge of Medicare and Medicaid guidelines.
• Must have a valid driver’s license and reliable transportation.
• Must have a current CPR certification.
• Excellent verbal and written communication skills.
• Basic computer skill proficiency. Ability to use Microsoft Windows.

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans.
• Develop and implement appropriate treatment plans based on patient needs and goals.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries.
• Maintain accurate and timely documentation of patient progress.
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care.
• Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes.
