Job summary

Connect Pediatrics is seeking a Pediatric Speech-Language Pathologist to join our in-home care team serving the Denver, Colorado area. You will evaluate, treat, and coordinate care for pediatric patients in their homes, collaborating with physicians, nurses, and a multidisciplinary team to support each child’s goals.

You will engage with families, provide progress updates, and contribute to a compassionate, collaborative culture focused on delivering comprehensive pediatric therapy in a home

Qualifications

• Active state Speech Therapy license in Colorado
• Current BLS/CPR certification
• Reliable transportation and driver's license with valid automotive insurance
• Pediatric therapy experience preferred but not required

Responsibilities

• Perform comprehensive evaluations using standardized tests for pediatric therapy needs
• Develop and implement treatment plans aligned with physician orders
• Provide in-home therapy services to pediatric patients across a range of diagnoses
• Engage with families with regular progress updates and goal communication
• Collaborate with a multidisciplinary team to create holistic treatment plans
• Maintain thorough documentation reflecting care provided and plan changes
• Participate in team meetings to review patient progress and refine strategies

Responsibilities

Perform comprehensive evaluations, using standardized tests to determine pediatric patients' needs for therapy.

Use your clinical judgment to develop and implement relevant treatment plans aligned with physician's orders.

Provide in-home therapy services for pediatric patients with a wide range of diagnoses.

Engage with patients' families, providing them with regular communication and updates on goals for their child.

Collaborate with a multidisciplinary team, including other therapy disciplines, physicians, and nurses, to develop holistic treatment plans that address the diverse needs of each patient

Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.

Actively participate in one-on-one meetings with team leads to discuss patient progress, share insights, and refine treatment strategies collaboratively.
