Job summary

Angels Care Home Health seeks a licensed Speech Language Pathologist to deliver therapy in patients' homes across Iowa. The role emphasizes rehabilitation of speech, language, swallowing, and cognitive disorders and requires a collaborative, patient-centered approach.

You will assess, plan, implement, and evaluate care, coordinate with the care team, and ensure compliance with Medicare guidelines. Must hold CCC-SLP, state license, and a valid driver's license.

Qualifications

• Master's degree in Speech Language Pathology required.
• Current state license in Speech Language Pathology.
• CCC-SLP certification from ASHA.
• Two years' experience in an acute care, rehab or home health setting preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide coordinated therapy in patients' homes implementing care plans.
• Plan, implement, and evaluate treatment to restore or improve communication and swallowing.
• Collaborate with interdisciplinary team to optimize patient outcomes.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competence from American Speech Language Hearing Association (CCC-SLP)

Current state license in Speech Language Pathology
