Job summary

Flagship Therapy is seeking a Speech-Language Pathologist in Commerce City, CO for full-time and PRN positions within our in-house rehab program. The role offers a competitive hourly pay rate and the opportunity to contribute to geriatric care and post-acute services.

Responsibilities include evaluation, planning, and delivering rehabilitation, with emphasis on patient-centered care and regulatory compliance. License and CCC credentials are preferred or required as stated.

Qualifications

• Active SLP license required.
• CCC credentials or eligibility preferred.

• Screen and evaluate patients with physical functioning disorders; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary interventions.
• Manage physical demands including standing, walking, lifting, transferring, and repositioning patients and equipment.
• Administer procedures within the rehabilitation plan (manual therapy, ambulation, therapeutic exercises, modalities, assistive devices).
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries ensuring regulatory and clinical compliance.

Irondale Post Acute - Commerce City, CO

Irondale Post Acute in Commerce City, CO is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking dedicated SLPs for our in-house rehab program!

Full Time and PRN Schedules: Flexible schedule with work-life balance

Pay: $45 - $55/hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitive, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Speech-Language Pathologist license is required.
