Willow Creek Lodge, affiliated with Ensign Services, is seeking a Speech-Language Pathologist for full-time and PRN schedules in Commerce City, CO. The role supports an in-house rehab program serving geriatric patients with a focus on screening, evaluation, and therapy in a post-acute setting.

Responsibilities

• Conduct screening and evaluation of patients and develop care plans.
• Deliver rehabilitative and medically necessary interventions.
• Assist with regular standing/walking and patient handling as needed.
• Administer rehabilitation procedures including manual therapy, ambulation, exercises, modalities, and wound care.
• Provide counseling to patients, families, and other providers.
• Prepare comprehensive discharge summaries in compliance with regulations.

Irondale Post Acute in Commerce City, CO is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking dedicated SLPsfor our in-house rehab program!

Full Time and PRN Schedules

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Speech-Language Pathologist license is required.

Open to Speech-Language Pathologists with Certificate of Clinical Competence credentials
