Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a full-time Speech Language Pathologist opening at Charleston Healthcare Center and Wayne Healthcare Center. You will work as an in-house team member, delivering therapy to residents and collaborating with the rehab staff.

Requires a Bachelor's and/or Master's in Speech-Language Pathology and a state license. You will evaluate, plan treatment, supervise swallowing and communication, lead meetings, and educate patients and

• Bachelor's and/or Master's in Speech-Language Pathology from an accredited program.
• Valid or pending State Speech Language Pathology License.
• Experience in rehab settings and interprofessional collaboration is a plus.

Responsibilities

• Evaluate, review, and screen patients' medical charts and evaluations under physician order; develop treatment plans.
• Plan, direct, or conduct rehabilitative treatment programs to improve communication, swallowing, and cognitive skills.
• Instruct residents, families, and caregivers to monitor speech and practice new skills; provide diet recommendations and swallowing techniques.
• Attend/lead interdisciplinary meetings and participate in family education sessions.
• Treat at multiple locations to meet residents' diverse needs.

Qualifications:

Must have a Bachelors and/or a Master’s Degree in Speech-Language Pathology from an Accredited Program.

Hold a valid or pending State Speech Language Pathology License

Responsibilities include but are not limited to:

Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.

Plan, direct, or conduct rehabilitative treatment programs to improve communication, swallowing, and/or cognitive problems, as well as administer, score, and interpret specialized speech tests.

Instruct residents, families, and caregivers to monitor speech and provide ways to practice new skills, diet recommendations, and modifications for safe swallowing techniques.

Attend/Lead interdisciplinary meetings as appropriate to relay clinical findings and make recommendations to other members of the rehab team, as well as participating in family meetings/education.
