Qualifications

• Valid State SLP License and Credentials.
• SNF and/or Acute care experience required.

Responsibilities

• Provide clinical services to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care aligned with patient needs and capabilities.
• Travel to nearby centers as needed.
