Job summary

Homeside Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to join our pediatric home health team in Hyde Park, MA 02136. You will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Active Massachusetts state license required with at least 1 year of related experience; strong documentation and communication skills are essential.

Qualifications

• Active Massachusetts State Speech-Language Pathologist License required.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings preferred.
• Strong documentation and communication skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide evidence-based therapy for speech, language, fluency, articulation, and voice disorders.
• Maintain accurate and timely documentation of care.
• Collaborate with clinical team to ensure coordinated treatment.
• Communicate effectively with office staff, patients, and caregivers.
• Collaborate with therapists, nurses, and physicians for comprehensive care.

Job Summary:
Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our pediatric home health team in the Hyde Park, MA 02136. In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Requirements

• Active Massachusetts State Speech-Language Pathologist License — required
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
• Basic computer literacy for documentation and communication
• Strong clinical and interpersonal communication skills
• Ability to work independently in a home health environment
• Reliable transportation for home visits preferred

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on patients’ needs and goals
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders
• Maintain accurate and timely documentation of patient care
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate effectively with office staff, patients, and caregivers
• Collaborate with healthcare professionals, including speech therapy assistants, nurses, and physicians, to ensure comprehensive care
