Coordinated Therapy Solutions is seeking a Speech Language Pathologist to evaluate, diagnose, and treat patients with speech or language disorders and to formulate individualized treatment programs.
You will work under a physician’s direction, assess residents, and develop care plans while administering tests and educating families on practicing new skills. This role requires licensure and a master’s/doctoral degree in Speech-Language Pathology.

The Speech Pathologist will evaluate, diagnose, and treat patients with speech or language disorders, formulate individualized treatment programs.
Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.
Under a physician’s direction, provides direct patient / resident treatment based on established plan of care.
Screens and evaluates Resident and develops appropriate care plan
Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
Administers, scores, and interprets specialized hearing and speech tests.
Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
Maintains patient records in an accurate and timely manner.
Devises special tools and treatments for the specific needs of patients.
Knowledge and skills to care for patient needs of the SNF population
Must possess problem solving skills to successfully perform job
Excellent verbal and written communication skills.

Strong customer service orientation.
Must be able to communicate effectively and professionally with other employees, patients, and family members
Ability to travel as needed to perform job
Graduate of an accredited Masters, or Doctorate in Speech language Pathology.
State license as a speech pathologist required.
Proficiency with all applicable state, federal, and professional standards
Prolonged periods of sitting at a desk and working on a computer.
