Total Care Therapy LLC in Avon, IN is seeking a part-time Speech-Language Pathologist to support residents in assisted living and independent living communities. You will evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders, developing individualized plans to improve quality of life.

This role offers competitive hourly pay ($60.00-$70.00) and per-visit compensation based on experience, with weekends optional.

Qualificações

• Current Indiana SLP license required.
• CCC-SLP certification preferred or eligible.
• CPR/BLS certification required.
• Valid driver’s license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred.

Responsabilidades

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
• Perform dysphagia evaluations and implement swallowing interventions.
• Reassess patient progress and adjust treatment plans as needed.
• Complete timely documentation in the EMR.
• Educate residents, caregivers, and staff on communication strategies and swallowing precautions.
• Collaborate with physicians, nursing staff, dietitians and the interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload delivering evidence-based therapy.

About the Role As a Speech-Language Pathologist, you will evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders while developing individualized treatment plans that improve residents' quality of life and functional independence.

Responsibilities

Evaluate residents and develop individualized speech therapy plans of care.

Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.

Perform dysphagia evaluations and implement appropriate swallowing interventions.

Reassess patient progress and adjust treatment plans as needed.

Complete timely and accurate documentation using the electronic medical record (EMR).

Educate residents, caregivers, and facility staff on communication strategies, swallowing precautions, and home exercise programs.

Collaborate with physicians, nursing staff, dietitians, and interdisciplinary team members to ensure coordinated, patient-centered care.

Maintain a productive clinical caseload while delivering compassionate, evidence-based therapy services.

Qualifications

Current State Speech-Language Pathologist (SLP) License

Current Certificate of Clinical Competence (CCC-SLP) preferred (or eligibility if applicable)

Current CPR/BLS Certification

Valid driver's license, reliable transportation, and auto insurance

Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
