Polaris Transitional and Extended Care Center in Anchorage, AK is seeking a Speech-Language Pathologist for a full-time role at a skilled nursing facility offering long-term care and post-acute rehab services.

The position provides a competitive pay range ($45.00 - $68.00 per hour) and a flexible schedule to support work-life balance. Open to new graduates with licensure to practice as an SLP.

Qualifications

• Speech-Language Pathologist license is required.
• Open to Speech-Language Pathologist at all experience levels. New Graduates are welcome.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative and medically necessary treatments in line with regulatory and clinical standards.
• Assist with patient transfers and use of devices; perform modalities and wound care as part of the plan.
• Provide consultation to patients, families, and caregivers.
• Generate discharge summaries ensuring regulatory compliance.

SPEECH-LANGUAGE PATHOLOGIST – FULL TIME

Polaris Transitional and Extended Care Center in Anchorage is a skilled nursing facility with long term care, post-acute rehab, and geriatric therapy services.

Pay Range: $45.00 - $68.00 per hour based on experience.

Full Time: Flexible schedule with work-life balance.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
