• Experience with differentiated instruction to create student-centered learning environments
• Ability to collaborate with district and outside agencies
• Excellent written and verbal communication skills

• Ensure compliance with State Performance Plan Indicators and applicable regulations.
• Develop and implement transition plans and ITP processes for students.
• Coordinate activities linking student goals to postsecondary plans.
• Provide on-the-job support and training for students.
• Maintain PEIMS attendance and reporting accuracy for off-campus assignments.
• Communicate post-secondary needs and objectives to families and agencies.
• Collaborate with teachers to support instruction for low-incidence populations.
• Present staff development on transition services.
