Saban Community Clinic seeks a Specialty Pharmacist to lead the Specialty Pharmacy Program and centralized refill services in Los Angeles. The role requires a PharmD, active CA pharmacist license, and experience in specialty pharmacy or ambulatory care. Strong collaboration with providers and accurate documentation are essential.

Responsibilities include patient onboarding, education, adherence monitoring, and program development to enhance patient outcomes within the clinic setting.

Qualifications

• Doctor of Pharmacy degree required.
• Current California Pharmacist license in good standing.
• Minimum of one year of pharmacist experience in specialty pharmacy or ambulatory care.
• PGY1 residency, California APh license, or board certification preferred.
• Able to work independently and with an interdisciplinary healthcare team.

Responsabilités

• Serve as the primary pharmacist for the Specialty Pharmacy Program.
• Coordinate specialty medication management from referral through follow-up.
• Onboard patients, educate, care plan, and reassess as needed.
• Monitor adherence, therapeutic response, and adverse effects.
• Collaborate with providers to optimize therapy and ensure compliance.
• Maintain documentation in EHR and pharmacy systems.
