Job summary

The Specialty Pharmacist role at Eskenazi Health is a key resource for delivering high-quality, evidence-based pharmacotherapy to patients on specialty medications through a centralized pharmacy model. The pharmacist educates patients, promotes adherence, and collaborates with prescribers and the care team to optimize outcomes.

This position requires PharmD (or equivalent), Indiana licensure or eligibility, and completion of a PGY1 residency or equivalent experience.

Qualifications

• PharmD required or BS in Pharmacy with equivalent experience.
• Indiana licensure or eligibility; license within 90 days of hire.
• ASHP-accredited PGY1 or 3+ years health-system/outpatient pharmacy.
• Basic Life Support certification required.
• CSP or related specialty credential preferred.

Responsibilities

• Clinical management of specialty medications and therapeutic drug monitoring.
• Educate patients and collaborate with prescribers and care teams.
• Ensure accurate documentation and transitions of care for patients.
• Promote safety and quality by identifying medication risks and adverse events.
• Support process improvements and staff training within the department.

Education

Doctor of Pharmacy (PharmD)

Indiana licensure or eligibility

ASHP PGY1 Residency or 3 years experience

Basic Life Support (BLS) certification

Board certification CSP preferred
