Job summary

Delta-T Group is seeking a Specialty Courts Case Manager in Center City Philadelphia. This on-site role requires travel between offices and various court rooms and police districts in the city, with hours 8:30 am–5:00 pm, Monday–Friday.

Responsibilities include coordinating services for chemically dependent program participants in the criminal justice system and conducting health screenings for warm hand-offs to treatment and other services.

Qualifications

• Bachelor degree required in criminal justice, psychology, social work, sociology or counseling.
• Minimum of two years in behavioral health services.
• Two years experience with adult addictions population.
• Minimum of two years in criminal justice system environments.
• Ability to engage adults dealing with addictions.
• Experience with public speaking.

Responsibilities

• Provide case management services to chemically dependent program participants involved in the criminal justice system.
• Complete social determinants of health/behavioral health screenings for warm hand-offs to treatment and other social services.
• Case management includes resource coordination and monitoring attendance at behavioral health treatment programs.
• Travel to police districts, Family Court, and the Juanita Stout Criminal Justice Center for court hearings and meetings with participants and providers.
• Monitor appropriateness of participants' level of care and advocate for changes as needed.
• Other duties as assigned.
