Job summary

The VA Texas Valley Coastal Bend Health Care System is seeking a Specialty Clinics Registered Nurse to join its clinical team. You will deliver patient-centered care using the nursing process, collaborate with the Teamlet, educate patients and families, and support self-management and Whole Health goals.

Responsibilities include triage, procedures support, documentation in CPRS, care coordination with multidisciplinary teams, and adherence to privacy and evidence-based care.

Qualifications

• ADN, BSN, or MSN must be ACEN/CCNE-accredited to qualify.
• Transcript documentation may be required with application.
• Federal employment experience and licensure compliance may apply.

Responsibilities

• Uses nursing process and evidence-based practice to collaborate with Teamlet and Team to meet patient care needs.
• Provides patient and family health education focusing on self-management, prevention, wellness, and Whole Health.
• Demonstrates advanced clinical knowledge in assessment, planning, implementation, documentation, and evaluation of care for a patient group.
• Educates patients on clinic procedures and expectations.
• Assists with clinic procedures, monitors for adverse effects, and triages care.
• Maintains privacy and confidentiality of patient information and documents securely.
• Promotes telehealth and non-face-to-face care once appropriate.
• Completes Nursing Notes in CPRS and closes encounters timely.
• Performs care management for Ambulatory Care Sensitive Conditions and coordinates with high-risk care teams.
