Qualifications

Responsibilities

• Provide clinical pharmacy services and medication therapy management for patients on specialty therapies.

Job description

We are seeking a hybrid Clinical Pharmacist to join a growing, closed door specialty pharmacy team. In this role, you will serve as a clinical partner to patients, caregivers, and providers, monitoring therapy outcomes, collaborating on individualized treatment plans, and guiding patients through complex specialty therapies. This position is a full time, direct hire opportunity with a hybrid schedule that includes regular remote work.

This role is well suited for a pharmacist who wants to practice at the top of their license and build meaningful, ongoing relationships with the patients they serve. Pharmacists from hospital, retail, ambulatory, infusion, and specialty backgrounds are encouraged to apply, as training in specialty therapies and workflows is provided. The ideal candidate is a strong communicator, clinically curious, and excited to help shape a pharmacy team during a period of growth.

Responsibilities:

• Provide clinical pharmacy services and medication therapy management for patients on specialty therapies, including oral, injectable, and cold chain medications
• Collaborate with physicians, nurses, and care teams to develop and optimize individualized treatment plans
• Educate patients and caregivers on medication use, adherence, and side effect management
• Monitor patient outcomes and make clinical recommendations to support therapeutic goals
• Serve as a dispensing pharmacist and support daily pharmacy operations
• Oversee pharmacy technicians through the prescription fulfillment process
• Support prior authorizations and help patients navigate insurance and access barriers
• Partner with manufacturers, payers, and patient assistance programs to support timely therapy access
• Maintain accurate documentation of clinical interventions and patient interactions
• Participate in a paid, phone based on call rotation for patients and providers outside business hours
• Obtain and maintain pharmacist licensure in additional states as needed

Qualifications:

• Doctor of Pharmacy (PharmD) from an accredited pharmacy program
• Active Pennsylvania pharmacist license in good standing
• Pharmacist experience in a hospital, retail, ambulatory, infusion, or specialty setting
• Willingness to obtain additional state pharmacist licenses
• Must reside within commuting distance of Pittsburgh, PA

Schedule:

• Full time, direct hire
• Monday through Friday, standard daytime hours, Eastern Time
• Hybrid schedule with regular remote weeks following an initial on site training period
• Paid on call rotation, primarily by phone from home, with a stipend plus additional pay for time worked
• Reliable high speed internet and a private home workspace required for remote work

Salary Range:

The salary range for this position is approximately $120,000 to $140,000 annually ($57.69 to $67.31 per hour), based on experience and qualifications.
