Job summary

Qualifications

• 3 years of specialty experience in care coordination.
• 5 years as a licensed RN with clinical experience and ability to learn a new specialty area.
• MN Registered Nurse (RN) License required.

Responsibilities

• Accesses, analyzes, plans and provides follow-up care.
• Facilitates timely referrals and fosters positive relations with community providers.
• Evaluates and documents status and follows patient progress through phone calls, tests and reports.
