Qualifications

Job description

Join to apply for the Specialized Foster Care Case Manager role at One Hope United.

Specialized Foster Care Case Managers are responsible for providing direct case management services to children considered specialized by the Department of Children and Family Services. Services are provided to birth families, foster families, and primary caregivers in various settings to ensure they receive appropriate services and continuity of care focused on resolving or preventing child abuse or neglect. The role includes assessment, planning, linkage, coordination, monitoring, evaluation, and advocacy for the child, family, foster family, and primary caregiver. The Case Manager collaborates with therapists, foster parents, primary caregivers, guardians ad litem, and other professionals, developing and maintaining relationships with mental health professionals, community agencies, and the child's support system.

Responsibilities
  1. Arrange visitation schedules based on visitation plans, supervise visitations, and document observations.
  2. Assist in providing information and accompany children to appointments as needed.
  3. Help primary caregivers and foster families implement tasks and activities per the case plan.
  4. Participate in professional development training as required by Illinois law and agency policies.
  5. Identify strengths, limitations, risk factors, and needs to coordinate services for the child and family.
  6. Serve as a communication conduit among all case parties and stakeholders.
  7. Conduct visits to assess child safety, permanency, and well-being.
  8. Develop and implement plans to meet the needs of children and families.
  9. Facilitate placement and service delivery collaboratively.
  10. Perform responsibilities in compliance with laws, policies, and procedures, within required timeframes.
  11. Attend court hearings, prepare and file documents, and provide testimony.
  12. Participate in after-hours on-call responsibilities periodically.
  13. Meet families in their homes at various hours, including evenings, nights, and weekends.
  14. Perform other duties as assigned.
Education, Experience, and Skills
  1. Bachelor’s degree in Social Work or a related Human Service field.
  2. Illinois DCFS Child Welfare Employee Licensure (CWEL).
  3. Willingness to undergo required trainings.
  4. Reliable transportation and willingness to drive daily.
  5. Availability for evenings, weekends, and on-call duties.
