Job summary

Memorial Health is seeking an Advanced Practice Provider (NP or PA) for a full-time, on-site position in Springfield, IL. The role focuses on evidence-based care for Wellness Center patients, including surgical and non-surgical weight loss and medication management.

You will collaborate with a multidisciplinary team, refer to other providers, and contribute to patient advocacy and education. Under physician supervision, you will coordinate department activities and support comprehensive care

Qualifications

• Certification as a Nurse Practitioner required or FNP as determined by the department.
• Must possess and maintain licensing as required by Illinois State law for APN.
• Valid CPR required; ACLS certification where required.
• Maintains DEA license where required.

Responsibilities

• Deliver and evaluate evidence-based comprehensive care to Wellness Center patients.
• Collaborate with Wellness Center multidisciplinary team; refer to other providers as needed.
• Coordinate department activities under physician direction; educate patients and families.

Position Type: On-site

Job Description Summary: Advance Practice Provider (Licensed Nurse Practitioner or Physician Assistant) Full Time M-F Day Shift (five 8's or four 10's). This position facilities the delivery and evaluation of evidence-based comprehensive care to Wellness Center patients. This includes surgical weight loss, nonsurgical weight loss, and weight loss medication management. Collaboration with Wellness Center multidisciplinary team is a major component of this role. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role. Under the direction physician or designee, provides coordination of their respective department or work area.

• Certification as a Nurse Practitioner required or FNP as determined by the department.
• Must possess and maintain licensing as required by Illinois State law for APN.
• Minimum of 1 year post-graduate school experience as an APN preferred.
• Valid CPR required. ACLS certification where required.
• Maintains DEA license where required.

Pay Transparency $0.01 - $0.01. Additional compensation in the form(s) of participation in our annual incentive program are dependent on role. Pay is based on factors such as work experience, education, certification(s), training, skills, and competencies related to the role.
