Title

Special Operations Case Manager / Nurse Case Manager (Hampton Roads, VA)

Role Overview

Provide full range of professional health nursing principles, practices, and procedures in clinical settings to analyze and manage problems associated with providing appropriate, cost‑effective care to Department of Defense beneficiaries.

Responsibilities
  • Collect, organize, record, and communicate data relevant to primary health assessments, including a detailed medical history, to develop time‑sensitive treatment plans that delineate the expected process of care delivery for selected case‑managed patients or populations.
  • Assess patients via telephone using established protocols to provide appropriate and cost‑effective care.
  • Establish priorities for patient care, monitor and evaluate progress toward stated goals to provide coordinated, efficient, and effective health care to beneficiaries.
  • Oversee discharge‑planning activities to ensure ideal timing and sequencing of patient care.
  • Provide professional assistance to health‑care finders in identifying patient needs for referrals to appropriate health‑care providers or facilities.
  • Negotiate, write, finalize, and administer product/service agreements to ensure compliance in meeting care goals.
  • Apply pre‑established utilization review criteria to recognize and report actual or potential quality and risk‑management issues for practice improvement.
  • Use VA/DOD Clinical Practice Guidelines to ensure evidence‑based, standardized care for military beneficiaries.
  • Understand the healthcare financial environment and fiscal aspects of population health to steward resources optimally.
  • Use research tools to critically evaluate existing practices, opportunities, and outcomes.
  • Manage information systems and technology to continuously improve practice and communicate outcomes to stakeholders.
  • Use computers and software applications for data collection, tracking, rendering, and analysis.
  • Design and present computer‑generated outcome analysis to provide timely, effective educational programs or project briefings.
  • Know pharmaceuticals, their effects, side effects, and complications to gauge impact on patient recovery.
Required Education, Experience, Skills, and Training
  • U.S. Citizenship required.
  • Secret or Top Secret clearance, or eligibility to obtain one.
  • Skill in understanding diagnoses to optimize patient function and self‑care.
  • Active, unrestricted RN or licensed clinical social worker license in any U.S. state, D.C., Guam, Puerto Rico, or U.S. Virgin Islands.
  • Minimum two years of nursing or case‑management experience; certification as a case manager by a recognized organization (e.g., Commission for Case Management Certification, American Case Manager Association, or American Nurse Credentialing Center) or obtain certification within one year of employment.
  • Knowledge of patient education principles and management of complex medical, psychosocial, and financial problems to enhance adherence to individualized treatment plans.
  • Seek and integrate peer and professional feedback to expand clinical knowledge and improve performance.
  • Effectively consult health‑care providers at all levels and negotiate with outside providers for services and products.
  • Possible requirement to receive and maintain clinical practice privileges.
  • Maintain credentialing requirements in good standing at a local MTF.
  • Fluent English communication skills for interacting with peers, agencies, vendors, providers, and MHS stakeholders.
  • Team coordination, facilitation, and communication skills to enhance goal achievement and effect change in system or policy.
  • Knowledge of local, state, and federal laws, regulations, and professional ethics.
  • Maintain current Basic Cardiac Life Support (BCLS) certification by the American Heart Association; provide documentation upon request.
Standard Company Requirements

  • Maintain regular and acceptable attendance.
  • Complete all required training.
  • Perform other assignments and duties as required.
