Job summary

Qualifications

• Master's degree in Social Work (MSW) is required.
• School of Social Work must be fully accredited by CSWE.
• Minimum 2 years of experience in case management, care coordination, and/or social work services.
• Licensed Clinical Social Worker (LCSW) license is highly preferred.

Responsibilities

• Provide medical social work services at the assigned facility.
• Coordinate case management and discharge planning with healthcare teams.
• Assist patients and families with resources, referrals, and support services.
