Job summary

Evolving Solution Services is seeking a Registered Nurse (RN) in Worcester, MA to oversee care for GAFC and AFC programs. The RN will collaborate with case managers and healthcare providers to assess needs, develop care plans, and monitor health status in community settings.

The ideal candidate holds an MA RN license (ASN or BSN preferred), with 1–2 years of nursing experience in home care, geriatrics, or community health. Driver’s license required; in-home visits are common.

Responsibilities

• Conduct initial and ongoing clinical assessments per MA regulations.
• Develop and update individualized care plans including ADLs.
• Perform in-home or site-based nursing visits per program requirements.
• Provide supervision and guidance to case managers and direct care staff.
• Train caregivers on medical tasks, infection control, and safety procedures.
• Monitor health status and identify changes needing intervention or escalation.
• Coordinate care with physicians, specialists, and therapists.
