Job summary

Doral Health & Wellness is seeking a Health Home Enrollment Specialist to enroll eligible patients in health home and community-based programs across New York. This field-based role requires five days on-site travel across the five boroughs, engaging with diverse populations and supporting walk-ins and referrals.

The Enrollment Specialist coordinates outreach, completes enrollment applications, and ensures documentation compliance with state and organizational standards.

Qualifications

• High school diploma required; Associate or Bachelor’s preferred.
• Prior experience in health home enrollment is preferred.
• Knowledge of Medicaid guidelines is a plus.

Responsibilities

• Conduct patient outreach and intake for health home programs.
• Educate patients and families about health home services, benefits, and eligibility.
• Document outreach, eligibility verification, and enrollment activities in the electronic care management platform.
• Complete enrollment applications and ensure documentation is compliant with regulations.
• Verify insurance coverage and Medicaid eligibility.
• Collaborate with case managers, care coordinators, and providers to activate services.
• Maintain organized records and update EHR systems.
• Track enrollment progress and report to program manager.
• Attend community events to conduct enrollments for eligible individuals.
