Job summary

Children’s Aid in New York, NY is seeking a Solution Based Case Planner to support families in addressing barriers to children’s health, safety, and well-being, through safety-focused assessments and intervention planning.

Qualifications

• Bachelor’s Degree in Social Work required.
• Experience with casework and advocacy is desirable.
• Strong communication and organizational skills.

Responsibilities

• Understand and promote the organization’s Mission, Vision, and Core Values.
• Maintain an average caseload of 12 families with a minimum of 2 contacts/month, 1 in-home.
• Complete all required documentation for SBC model and ACS.
• Provide services including family and individual therapy, advocacy, educational monitoring, and general case management.
• Receive training and certification in the SBC model and ensure fidelity.
• Collaborate with and refer to appropriate service providers for case coordination.
